Rescue and nursing care of a neonate with severe laryngomalacia complicated with breath-holding attack

To summarize the rescue and care of a case of breath-holding attack caused by congenital contractures of limbs and face,hypotonia,and developmental delay(CLIFAHDD) syndrome. Key points of nursing:early identification and intervention of breath-holding attack in CLIFAHDD syndrome,and implementation of rapid reactive rescue;implementing individualized developmental care measures to avoid stimuli that induce breath-holding attack;paying attention to the assessment of oral feeding ability,strengthening the exercise of oral feeding ability and nutrition management;strengthening perioperative diversified airway management to prevent postoperative complications;the monitoring of early neurological and motor system development,and early start of rehabilitation care;doing a good job in health education for caregivers to help them master the first aid treatment and growth and development assessment methods in the event of a breath-holding attack. After 53 days of hospitalization,the patient was discharged. After being discharged from the hospital for more than 1 month,the patient had breath-holding attack 2~4 times a day. After being discharged from the hospital for more than 2 months,the patient had no breath-holding attack,and only had occasional convulsive seizures during sleep,which could be relieved spontaneously.

Current situation of the emotional labor in pediatric nurses and its influencing factors analysis

Objective To investigate the status and influencing factors of the emotional labor of pediatric nurses,so as to provide references for improving their emotional labor. Methods Totally 520 pediatric nurses were selected from 8 hospitals in Suzhou in July 2022. Under the guidance of the Conservation of Resources Theory,they were investigated with the general information questionnaire,Emotional Labor Scale for Nurses,Spiritual Climate Scale and Nursing Work Environment Scale. Multiple linear regression was performed to analyze the factors affecting the emotional labor. Results 443 valid questionnaires were collected. The score of the Emotional Labor Scale was[64.00(60.00,68.00)],and the scores for emotional control effort in profession,patient-focused emotional suppression and emotional pretense by norms were[30.00(28.00,34.00)],[20.00(18.00,21.00)] and [14.00(12.00, 15.00)]. The results of multiple linear regression analysis showed that total working time,work title,nursing work environment and spiritual climate were the main influencing factors of each dimension of the emotional labor(P<0.001). Conclusion The emotional labor of pediatric nurses is at an upper-middle level. The nursing managers should pay attention to the emotional labor of pediatric nurses,optimize the nursing work environment and create good working atmosphere.

Positive psychological experience of stroke patients:a qualitative meta-synthesis

Objective To analyze and evaluate the positive psychological experience of stroke patients to provide reliable bases for their psychological nursing. Methods Chinese and English databases,such as PubMed,Web of Science,Embase,CINAHL,PsycINFO,CNKI,Wanfang Database,VIP,and CBM,were searched,and the literature on the qualitative research on the inner positive psychological experience of stroke patients was retrieved. The time limit of the retrieval was from the establishment to April 2021. The literature quality was evaluated using the 2016 Australian JBI Evidence-Based Health Care Center Quality Evaluation Criteria for Qualitative Research,and the results were consolidated using meta integration method. Results A total of 14 studies were included,and 65 results were extracted and summarized into 12 new categories,which were combined into 3 integrated results. (1)Positive coping attitude and psychology:accepting and adapting to stroke,feeling lucky and grateful,optimistic and positive to face the situation,strong spiritual strength,goals and hope for the future;(2)Positive changes after stroke:increasing health awareness,more intimate and harmonious interpersonal relationships,personal growth,new thinking about the philosophy of life;(3)Support factors of positive psychology;positive coping strategies at the individual level,social support from family,friends and peers,social support from the medical system. Conclusion Existing evidence suggests that patients with cerebral apoplexy after stroke will have a positive psychological experience and change. Nursing staff should pay more attention to the patient’s psychological state and the support of the positive psychology factors,meet their psychological needs,help patients strengthen social support system,and maintain a positive mental state,in order to promote patients to recover and improve the quality of life.

Objective To systematically evaluate the disease experience and psychological feeling of young and middle-aged stroke patients,so as to provide theoretical bases for medical staff to meet the needs of patients,improve the quality of nursing services,and further improve the rehabilitation treatment effect and mental health of the population. Methods The computer retrieval was performed in PubMed,Embase,Web of Science,Cochrane Library,ProQuest,CBM,CNKI,WanFang Data,VIP for the qualitative research on real experience,psychological experience and feeling of young and middle-aged patients with cerebral apoplexy,with the retrieval time from inception to July 2020. The literature quality was evaluated using the qualitative research quality evaluation criteria of the Joanna Briggs Institute Evidence-based Health Care Center in Australia,and the research results were summarized and integrated using the pooled integration method. Results A total of 14 articles were included;90 themes were extracted;9 categories were summarized;3 integrated results were formed,namely breaking the normal life,seeking attention and support,getting a positive experience. Conclusion Nurses should pay attention to the negative psychological status of patients,unite family and social support to help patients build confidence and return to family and society. They also need to improve their professionalism,formulate targeted rehabilitation plans for patients,improve continuous nursing services,and provide timely professional support for patients. Special attention should also be given to the positive psychological experience of patients,so as to assist patients to maintain a positive state,and achieve a virtuous circle.
